Who needs a physical?
Freshmen and transfer students are required to submit complete physicals prior to beginning classes.
Athletes are annually required to submit complete physicals prior to beginning practice.
Sophomores, juniors and seniors who are not participating in sports are not required to complete physicals annually.

Who needs a TDaP Booster?
Everyone (IDPH TDaP Letter from Governor Quinn)
The State of Illinois is now requiring all students entering 9th grade to have recent TDaP booster.
Starting in the fall of 2013, the State of Illinois is requiring that all high school students have a recent TDaP booster, so if you are scheduling a physical or a sports physical, go ahead and ask for one.
Who needs a Medical Authorization Form?
Students taking any medication, either prescription or non-prescription, during the school day, including self-administration of asthma inhalers, epi-pens or insulin. This form needs to be completed only once for non-prescription medication and annually for prescription medication. It must be signed by the prescribing physician and parent. Submit the form and properly labeled medication to the Health Office.
